The 49ers will meet the Baltimore Ravens in New Orleans in two weeks.
In front of their home crowd, the Falcons were on fire in the early part of the game, racing to a 17-0 lead on the strength of a pair of touchdown passes from Matt Ryan to Julio Jones.
The 49ers rallied behind quarterback Colin Kaepernick, who didn't have the same day running the football as he did against the Green Bay Packers last week, but he was very efficient, completing 16 of 21 passes for 233 yards and a touchdown.
Frank Gore added 90 yards rushing and a pair of scores, including the go-ahead touchdown in the fourth quarter.
It will be the 6th appearance in the Super Bowl for the 49ers, but their first in nearly 20 years. They are 5-0 in their previous 5 trips to the NFL's championship game.
You can watch Super Bowl XLVII on Local 5 on February 3rd
